By scheduling or receiving services from Southlake Mobility & Rehab, you ("Client") agree to the following Terms and Conditions. These terms are subject to change without notice.

1. Services Provided

Southlake Mobility & Rehab ("Company") provides therapeutic services, including but not limited to Fascial Stretch Therapy (FST) and Corrective Exercise. All services are designed to improve mobility, reduce pain, and enhance functional movement.

​

2. Eligibility

Clients must be 18 years or older or have written consent from a legal guardian. The Company reserves the right to request a physician's clearance prior to beginning services, if deemed necessary.

3. Session Scheduling and Attendance

All sessions are by appointment only. Clients are responsible for arriving on time. Late arrivals may result in shortened session time without a discount or refund.

4. Cancellation and No-Show Policy

Cancellations must be made at least 24 hours in advance of the scheduled session. Cancellations made with less than 24 hours' notice or no-shows will result in the forfeiture of that session and will not be refunded or rescheduled.

5. Payment Policy

Payment is due in full at the time of booking or prior to the session. Packages and promotions, if offered, are non-refundable and expire by the noted date.

6. Refund Policy

All sales are final. Southlake Mobility & Rehab does not offer refunds for completed or missed sessions. Any exception is solely at the discretion of the Company.

7. Assumption of Risk and Liability Waiver

By participating in any service, the Client acknowledges that Fascial Stretch Therapy and Corrective Exercise may involve physical exertion and carries some inherent risks. The Client agrees to assume full responsibility for any injuries or damages that may occur and releases Southlake Mobility & Rehab and its practitioners from any liability.

8. Right to Refuse Service

Southlake Mobility & Rehab reserves the right to refuse or discontinue service to any individual at any time for any reason, including but not limited to inappropriate behavior, safety concerns, or non-compliance with these terms.

9. Confidentiality and Privacy

All client information is kept strictly confidential and will not be shared with third parties without written consent, except as required by law.

10. Changes to Terms

Southlake Mobility & Rehab may update these Terms and Conditions at any time. Continued use of services following any changes indicates acceptance of those changes. By scheduling a session, you acknowledge that you have read, understood, and agree to the above Terms and Conditions.
